• Name
• Date of Birth
• Current Mailing Address 
• US Tax Identification Number (ITIN)

2) Information about the rental property:

• Address of Rental Property
• Purchase Date and Purchase Price
• Number of days that you rented the property 
• Number of days you vacationed in the rental property.

3) Revenue and Expenses.

4) Days spent in the US for each year in the past three years.
